It is extremely helpful if agencies could also request that the client completes the eligibility questionnaire below and asks the client to send into the Bureau evidence of their income in the form of wage slips, benefit notification letters and bank statements. Completion of this questionnaire can considerably speed up the time it takes for us to offer the client an appointment.
